Safety Letter

Child Medical Statement for Child Care -The physical is good for only one year. This form must be completed in full, including the date of the physical, all immunization dates, and signed by the doctor or nurse practitioner. If your child is a returning student, then we can use their current physical that is on file and dated. This form is due 30 days after the start of school.
